Rapport’s bipolar trial clears a key hurdle, but the size of the benefit draws scrutiny

Rapport Therapeutics reported that its Phase 2 trial of RAP-219 for acute bipolar I mania met its primary and key secondary endpoints. Among 253 adults treated for three weeks, the drug improved Young Mania Rating Scale scores by 15.4 points, versus 12.3 points with placebo—a 3.1-point placebo-adjusted difference (p=0.045).

The result was statistically significant but below the 4-point improvement Rapport and some analysts had previously identified as a clinically meaningful benchmark. The modest margin over placebo leaves uncertainty about how strong the effect will prove in larger trials.

Analysts nevertheless viewed the readout as a positive proof-of-concept result, particularly because improvement increased over the three weeks and was seen across all 11 rating-scale items. Some said the 3.1-point effect was within the range of approved treatments; others emphasized that expectations for the trial had been low.

Rapport plans to discuss the results and trial design with the FDA in early 2027, with Phase 3 studies expected to begin in the second half of 2027. Analysts see bipolar mania as a potentially significant expansion beyond the company’s epilepsy work, while noting that larger studies and longer-term safety data will be important.
