FDA proposes a more consistent—but tailored—approach to rare-disease drugs

On October 14, 2026, the FDA announced its Rare Disease Framework Initiative, a plan to rethink how rare-disease drugs and biologics are developed and reviewed.

The initiative aims to make regulatory decisions more consistent and predictable while tailoring evidence expectations to each disease, patient population, treatment, and the evidence that can realistically be collected in small populations.

Planned priorities include involving patients earlier, drawing on outside disease experts, using innovative scientific and statistical methods, and applying risk-based approaches to areas such as manufacturing review.

This is a framework-development effort, not a set of new requirements already in force. The FDA says the initiative will preserve the statutory standards for safety and effectiveness.

The agency planned a public meeting for October 28, 2026, and further public input to help shape the framework.
