AbbVie and Genmab reported a Phase 3 win for Epkinly (epcoritamab) plus standard R-CHOP chemotherapy in newly diagnosed diffuse large B-cell lymphoma (DLBCL). In the EPCORE DLBCL-2 trial, the combination reduced the risk of disease progression or death by 51% versus R-CHOP alone (hazard ratio 0.49).
The result could position Epkinly as the first bispecific antibody approved for first-line DLBCL, but it is not approved for this use yet. The companies plan to discuss the data with regulators; fuller results are still awaited.
Whether it will be the best option is an open question. The reported result is promising, but comparisons with competing first-line approaches—including Roche’s Polivy-containing regimen—will depend on detailed trial data, safety, durability, and direct or carefully adjusted comparisons.
The announcement has drawn attention because a first-line approval could change treatment practice and intensify competition in DLBCL. For now, the evidence supports a potentially important new option, not a settled ranking of treatments.
