Eli Lilly’s recent Q3 2026 results coverage highlights cash-pay sales as an important route for its obesity drugs, alongside insurance and government access programs. Reuters reported that roughly 30% of U.S. Zepbound prescriptions were cash-pay, citing IQVIA data shared by an analyst.
Lilly executives said the company had launched in the UAE, UK, Saudi Arabia and Mexico, and expects cash-pay weight-management sales to be an initial focus in many additional markets, where reimbursement can take longer to establish.
The strategy brings a trade-off:
strong demand is boosting sales, but Lilly has lowered U.S. cash-pay prices, contributing to lower realized prices even as prescription volume grows.
Lilly also described low out-of-pocket costs as helping patients start and stay on treatment, including through the Medicare GLP-1 Bridge program.
