Ionis–Otsuka ALS Success and Boehringer’s Potential $1B Oncology Deal

Ionis Pharmaceuticals and Otsuka reported positive Phase 3 FUSION results for ulefnersen, an investigational antisense medicine for FUS-ALS—a rare ALS subtype caused by mutations in the FUS gene.

The trial met its primary endpoint:
ulefnersen significantly improved a prespecified composite of functional impairment and survival versus placebo (p=0.0005). The analysis combined ALSFRS-R change, time to rescue, and time to death or permanent ventilation.

Secondary measures also favored ulefnersen, including serum neurofilament light-chain levels and time to death, permanent ventilation, rescue, or withdrawal because of disease progression. Most adverse events were reported as mild to moderate.

The trial enrolled 89 patients, with 73 included in the primary analysis. FUS-ALS represents roughly 0.6% of ALS cases and currently has no approved mutation-targeted therapy.

Otsuka and Ionis said they will discuss the results with the FDA and other regulators regarding expedited submissions. Otsuka also announced a global early-access program for eligible, genetically confirmed patients. The drug remains investigational and is not yet approved.

Boehringer Ingelheim and Envisagenics signed a multi-year research collaboration and option agreement focused on tumor-specific targets derived from alternative RNA splicing for precision treatments of solid tumors.

Envisagenics could receive more than $1 billion in aggregate payments, including upfront funding, research support, option fees, development and regulatory milestones, commercial milestones, and royalties. The headline value is contingent—not an immediate $1 billion payment.

The partnership combines Envisagenics’ RNA-splicing and computational drug-discovery platform with Boehringer’s oncology development capabilities; the companies must first validate targets and exercise development options before potential products emerge.
