Germany pushes through healthcare reform package despite pharma's drug discount resistance

Germany's Bundestag has passed a major healthcare reform and cost‑containment package aimed at stabilising statutory health insurance (GKV) finances and cutting total healthcare spending by roughly €16–20 billion from 2027 onward, despite fierce resistance from the pharmaceutical industry.

The new GKV‑Beitragssatzstabilisierungsgesetz (GKV‑BStabG) tightens drug‑pricing rules, extends the vaccine price moratorium, and raises the mandatory manufacturer discount (Herstellerabschlag) to 15.5% for patented vaccines, while other branded medicines also face higher rebates and stricter discount obligations.

In response to industry fears around unpredictable variable discounts on medicines, the government dropped the variable‑rebate scheme and replaced it with a fixed discount rate, which pharma groups say will still pressure margins but at least be easier to plan for.

Drug companies and investors have warned the reforms could reduce R&D investment and innovation in Germany; several large firms have publicly threatened or announced cuts to planned German investments, yet the government explicitly stated that the sector will not be exempt from cost‑control measures.

The package also includes broader health‑system changes such as higher patient co‑payments for prescription medicines, tighter limits on hospital cost increases, and reforms to co‑insurance of spouses, with the overall goal of stabilising contribution rates and preventing future premium hikes in the statutory system.
